2026 Quality Award

In 2026, for the second consecutive year, Integrated Health Collaborative (IHC) was recognized by Anthem in Elevance Health’s annual Care Provider Recognition Program. IHC was among just 45 medical practices and health systems in the United States to receive this distinguished honor.

The program recognizes primary care providers who demonstrate exceptional performance in delivering high-quality, coordinated care to members enrolled in Elevance Health’s employer-sponsored, Medicaid and Medicare Advantage health plans.

Providers selected for this honor rank among the top 1% of providers participating in Elevance Health’s national value-based care network. Selections are based on key performance measures, including preventive care screenings, immunization rates, chronic disease management, medication adherence and more than a dozen additional quality of care measures. This achievement reflects the commitment of IHC’s physicians, advanced practice providers, nurses, care coordinators, and clinical support teams to delivering exceptional patient care.
