Who we are

In May 2013, the Aultman Health Foundation leadership appointed a 20-physician steering committee from across the health system to develop a clinically integrated network (CIN) for Stark and surrounding counties. The steering committee developed a governance model and created the network named Integrated Health Collaborative. Integrated Health Collaborative has partnered with many of the local hospitals in the Independent Hospital Network:

  • Alliance Community Hospital
  • Aultman Hospital
  • Aultman Orrville Hospital
  • Joel Pomerene Hospital

Integrated Health Collaborative officially formed in December 2013 with the nomination of the Board of Directors.

As health care transforms from volume-based reimbursement to value-based care, Integrated Health Collaborative will engage physicians to lead the transformation to deliver high-quality, patient-centered, efficient care. Integrated Health Collaborative will lead the development of physician initiatives focused on population health management.

Integrated Health Collaborative seeks to accomplish three goals, known as the “Triple Aim.”

  1. Improve the patient care experience.
  2. Improve population health.
  3. Reduce the per capita cost of care.
